 What is Nursing Process?

The Nursing Process is a scientific, systematic, and patient-centered approach to provide care.
The nursing process is a step-by-step method that helps nurses identify patient problems, make a diagnosis, plan goals, implement care, and evaluate results. It ensures that patient care is not random but organized, safe, and effective.

The 5 Steps of Nursing Process

1. Assessment

The first step is collecting complete information about the patient.

Nurses collect data by observation, interview, and physical examination.
नर्स patient की health history, lifestyle, emotional condition और vital signs (BP, temperature, pulse) note करती है।

Example: If a patient comes with fever → nurse records temperature, checks dehydration, asks about headache or cough.

2. Diagnosis

This is about identifying patient’s problems based on the collected data.
Remember: Nursing diagnosis ≠ Doctor’s diagnosis.

Doctor’s diagnosis → Disease name (e.g., Pneumonia).
Nursing diagnosis → Patient’s response (e.g., difficulty in breathing, weakness, risk of high fever).

3. Planning / Goal Setting

In this step, the nurse makes a care plan with short-term and long-term goals.  Goals must be SMART (Specific, Measurable, Achievable, Realistic, Time-bound).

Example:  Short-term:  Fever will reduce within 24 hours.
Long-term:  Patient will regain strength within 1 month.

4. Implementation

This step is about taking action on the care plan. Implementation का मतलब है जो plan बनाया गया है उसे practically लागू करना।

  1. Giving prescribed medicines
  2. Monitoring vitals
  3. Providing oxygen therapy or injections
  4. Educating patient & family
  5. Emotional support

5. Evaluation

The final step is to check results.

  1. Did the goals succeed?
  2. Has the patient’s condition improved?
  3. If not, the plan is revised.

Example: If fever reduced and breathing improved → plan successful
If not → nurse changes care plan.
